Overview
Providing NHS services as an associate in England involves balancing the independence of self-employment with the responsibilities of delivering care on behalf of a practice owner.
As a self-employed dentist working within a practice owned by another dentist or a dental company, you bring clinical autonomy to your role, while the practice provides the premises, equipment, administrative support, and overall framework within which you work. When both parties understand their rights and responsibilities clearly, the arrangement can offer significant benefits. Patients experience continuity of quality care, practices can maintain stable service delivery, and associates have the opportunity to develop their skills and build a sustainable income.
Your relationship with the practice owner is governed by your associate agreement. A well-constructed agreement sets out the basis on which you provide NHS services, including how activity is allocated, how you are remunerated, and what obligations you must meet under the relevant General Dental Services (GDS) or Personal Dental Services (PDS) contract. This is particularly important because, while you are not a party to the NHS contract itself, you are performing work that contributes directly to the practice’s fulfilment of its contractual duties. Clear provisions help ensure that both you and the practice understand how responsibilities are shared, including record-keeping expectations, engagement in quality improvement, and the handling of complaints or governance requirements.
Self-employment offers autonomy and flexibility, but it also requires careful attention to financial management, indemnity arrangements, pension choices, and tax responsibilities. Associates must be aware of how NHS payments flow through the practice, how deductions are applied and how performance measures may affect income. Working arrangements within the practice, including diary management, access to facilities, staff support and adherence to local policies, also form essential parts of day-to-day practice life.
