You must inform the CQC if changes at your practice affect your CQC registration – for example, when you:
- Change your name or registered address
- Add a practice at another location
- Add or remove a partner (if registered as a partnership)
- Add or remove a director (if registered as a company/organisation)
- Change from one legal entity (sole trader, partnership or organisation) to another
- No longer provide regulated activities or a provider has died.
