Overview
The funding framework for NHS dental services in Wales underwent a significant change which took effect from April 2026. The model that had persisted for almost two decades was changed. Pre-April 2026, the contract operated in a significantly different way. The previous system was widely criticised for encouraging volume of delivery over preventative care and clinical autonomy. It was also believed that the system did not accurately reflect the complexity of the needs that were presented by certain patients.
The Welsh Government introduced the April 2026 changes in part to address the concerns surrounding the former Units of Dental Activity (UDA) model by changing the way contracts are remunerated. The updated contract now uses a payment matrix, which has been segmented to reflect six key performance indicators which are recall and monitoring, urgent treatment for new patients, new patient assessments, care packages, prevention payments, and local or national priority projects.
At the time of writing this advice we are awaiting confirmation of various parts of the guidance that have been provided by Welsh Government, such as a final version of the vignettes document and how some of the metrics will be measured. The below advice has been compiled based on everything that we know so far. By understanding the new remuneration model, you will be better equipped to manage your NHS obligations and patients under the updated contract.
